Preschool Registration for 2010 - 2011
Preschool Registration for the upcoming school year begins in late January, 2010.
Registration packets w/$40 registration fee (non-refundable) are accepted at this
time. Confirmation of class ENROLLMENT depends on receipt of May 2011 deposit no
later than August 1, 2010.
NOTE: If this deposit is not paid by the August 1 deadline, your child will forfeit
their fall enrollment with no additional notice from the preschool.
REGISTRATION PACKET: include $40 Registration fee made out to Weekday Ministries.
(Place all children from the same family in the same envelope to be processed together,)
- Weekday Ministries/Preschool Registration form (available at preschool office only)
- O.D.J.F.S. Child Enrollment and Health Information Form (available at preschool office or here )
- O.D.J.F.S. Medical Statement Form: (available at preschool office or here)
- Children currently enrolled need only keep medical form from expiring.
- New children need a medical form on file when they start school in September/deadline 30 days from first day of attendance.
MAY 2011 TUITION (non-refundable) & SUPPLY FEE: due by August 1, 2010. You will receive a bill in July, 2010.
Checks are made out to Weekday Ministries with your child’s name in the memo.
Ask about the 5% discount when you pay the years tuition in full.
