Preschool Registration for 2012 - 2013

Preschool Registration:
Pick up Weekday Ministries/Preschool Registration form (available at the preschool office only) along with $40 Registration fee (non-refundable) made out to Weekday Ministries then:
turn into preschool office (Entrance #2 at Christ's Church at Mason) during office hours, Monday thru Friday 8:30AM - 4:00PM

Confirmation of Enrollment: You will receive a bill in June 2012 for May 2013 tuition (non-refundable) & $35 supply fee. Receipt of this payment along with the following O.D.J.F.S. forms by August 1, 2012 deadline is required to guarantee enrollment for the 2012-2013 school year.

O.D.J.F.S. Child Enrollment and Health Information Form (available at the preschool office or here)
O.D.J.F.S. Medical Statement Form (available at the preschool office or here)

• NEW students need a medical form on file within 30 days of first day.
• CURRENT students need only keep medical form from expiring.

NOTICE: If this deposit and paperwork is not received by August 1 deadline, you will forfeit your child’s fall enrollment with no additional notice from the preschool.
